Please ensure all paperwork is entirely completed and signed by the property owner if applicable. This will help streamline our ability to process your application in a timely fashion.
All applications and supporting documents can be emailed to PCHD@portagehealth.net or directly to your health inspector.

Payment (if applicable) is due at the time an application is submitted.
We accept payments by credit card over the phone once we have received all the appropriate paperwork. Transaction fees apply.
