Community Health Assessment

The Community Health Assessment is a comprehensive look at the health and well-being of Portage County residents. The purpose of compiling this community health assessment is to identify the needs of the community in order to provide appropriate services.

This data-rich assessment provides a comprehensive look at the health and well-being of Portage County. Health-related data includes adults (19 years and older), Youth (ages 12-18), and children (ages 0-11) who participated in a county-wide health assessment survey.

Portage County completed its first Community Health Assessment in 2016. In 2019, a second Community Health Assessment was completed. The Community Health Assessment for Portage County is the first step necessary for the development of a Community Health Improvement Plan.

The Portage County Community Health Assessment would not exist without the assistance of community leaders and many public and private health partners, whose members took the time to carefully plan and carry out the assessment. A special thank you to the local school officials who assisted in the assessment process and set aside valuable time that allowed 6th to 12th grade students to participate in this important project.
