Chip Reports

The Community Health Improvement Plan (CHIP) is a three-year systematic effort to address public health problems that are identified by consensus from community stakeholders using data from the Community Health Assessment.

 

The Community Health Improvement Plan provides guidance to our agency, its partners, and its stakeholders, on improving the health of the population within the county. The Portage County Health District staff provided guidance to lead community partners toward consensus and the ultimate identification of three priorities areas and three cross-cutting factors. The Portage County Health District is committed to improving the health of our residents of Portage County and maintains the Community Health Improvement Plan.

 

The Portage County Community Health Improvement Plan was released on January 18, 2023.
