Measles cases have been reported in Ohio! Here’s what you need to know.

The Portage County Combined General Health District recommends administration of all age-appropriate vaccines to prevent disease. 

VACCINE CLINIC DATES, TIMES, AND LOCATIONS

  • Clinic Hours: Every Wednesday, 8:30 AM – 12:00 PM and 1:00 PM – 4:00 PM
  • Appointments outside of regular business hours can be scheduled. Appointments after 4:00 PM may be accommodated upon special request. PCHD is closed on government holidays.
  • Ask us about other programs available: Car Seats, Cribs for Kids, Prevention, Retention, and Contingency (PRC), Project DAWN (Narcan), Safe Communities (traffic safety), Sexual Health (condoms), and WIC!

  The vaccine clinics follow the recommended immunization schedules (see below) that are approved by the Centers for Disease Control and Prevention (CDC) and the Advisory Committee on Immunization Practices (ACIP).

BILLING AND PAYMENT INFORMATION

We accept the following insurances: United Health Choice Plus, Medical Mutual, Mutual Health, SummaCare, Aetna, Anthem Blue Cross/Blue Shield, Cigna, Medicaid, Molina, United Health – Community Plan, Care Source, Buckeye and Medicare Part B.  HMOs are not accepted-Those with HMOs need to go to an HMO provider. For those who are uninsured or under insured, there is a $10 administrative fee per vaccination – cash, check, or credit card accepted.

No person shall be denied vaccines that are provided free to PCHD by the Ohio Department of Health for failure to pay an administration fee or to make a donation.
