The vaccine clinics follow the recommended immunization schedules (see below) that are approved by the Advisory Committee on Immunization Practices, the American Academy of Pediatrics, the American Academy of Family Physicians, and the American College of Obstetricians and Gynecologists.

BILLING AND PAYMENT INFORMATION
We accept the following insurances: United Health Choice Plus, Medical Mutual, Mutual Health, SummaCare, Aetna, Anthem Blue Cross/Blue Shield, Cigna, Medicaid, Molina, United Health – Community Plan, Care Source, Buckeye and Medicare Part B. HMOs are not accepted-Those with HMOs need to go to an HMO provider. For those who are uninsured or under insured, there is a $10 administrative fee per vaccination – cash, check, or credit card accepted.
No person shall be denied vaccines that are provided free to PCHD by the Ohio Department of Health for failure to pay an administration fee or to make a donation.
TB Skin tests are $20 each – cash, check, or credit card accepted.
