The Community Health Improvement Plan (CHIP) is a three-year systematic effort to address public health problems that are identified by consensus from community stakeholders using data from the Community Health Assessment.
The Community Health Improvement Plan provides guidance to our agency, its partners, and its stakeholders, on improving the health of the population within the county. The Portage County Health District staff provided guidance to lead community partners toward consensus and the ultimate identification of three prioritiy areas and its cross-cutting factor. The Portage County Health District is committed to improving the health of our residents of Portage County and maintains the Community Health Improvement Plan.
