This 2024-2026 Strategic Plan reflects the charting of the future course for the Portage County Health District over the next three years; establishing what we hope to achieve, how we will achieve it, and how we will know we have achieved it. The Strategic Planning Team meets quarterly to assess the plan and implement changes as necessary.
